By submitting this form, you acknowledge and understand that:

This form is an expression of interest only and does not guarantee participation in the Program.
Activity Mentors may be subject to a screening and approval process before being invited to participate.
Activities may include cultural, educational, outdoor, agricultural, artistic, culinary, or community-based experiences.
Activity Mentors are expected to provide a safe, respectful, and welcoming environment for participants.
Participants are adults aged 18 years or older.
Activity details, scheduling, compensation (if applicable), responsibilities, and program expectations will be discussed and agreed upon separately before any activity is confirmed.
Activity Mentors are expected to comply with applicable Japanese laws, regulations, and safety requirements relevant to their activities.
The Program Operator reserves the right to approve, decline, modify, or discontinue activities where necessary for safety, operational, or program-related reasons.
Submission of this form does not create an employment, partnership, or contractual relationship between the Activity Mentor and the Program Operator.
